Telophase is the last stage of mitosis. During this stage, the chromosomes uncoil and form two nucleus in opposite sides of the cell. Each nucleus receives one set of chromosomes. A new nuclear envelope forms around each nucleus, new nucleoli are formed, and the spindle fibers break down. Additionally, cytoplasm of the cell begins to divide, and a cell plate forms down the middle that will eventually divide the cell into two daughter cells.
